Our model of care is based on an integrated approach whereby family, service users, commissioners and carers are all involved in the rehabilitation and recovery of the individual. Our secure services are therapy and recovery focused, providing assessment, treatment and rehabilitation for both men and women. The aim is to effectively progress service users though the secure care pathway, into the least restrictive environment possible while working to reduce the possibility of readmission back into secure care.
Degrace Health Care’s comprehensive network of integrated care pathways means we can support individuals to move through their recovery journey from medium to low secure, and onto specialist step-down rehabilitation services as required.
This approach helps to shorten the length of time an individual stays within services, improving the experience for the service
user and enhancing value with NHS commissioners. We are able to provide a comprehensive prison transfer assessment service for prisoners, including those who have yet o be sentenced and who need an assessment and clinical recommendation for the courts
